This package offers a more comprehensive and detailed framework, addressing a wider range of physical, emotional, mental, and spiritual needs, while ensuring that your directives are fully aligned with your values, beliefs, and preferences. What's Included: Extended Initial Consultation (90 minutes): A longer, in-depth conversation with a compassionate care consultant, where we explore your wishes in greater detail. We will address your physical care preferences, emotional and psychological needs, spiritual or religious considerations, and any specific desires for your final days. This session allows us to better understand your life philosophy, ensuring that every aspect of your directive is a true reflection of who you are. Comprehensive Advanced Care Directive Creation: We will create a customized, detailed advanced care directive that includes: Pain and Symptom Management: Tailored strategies for controlling pain, anxiety, and other symptoms. Life-Sustaining Treatments: Clear instructions on your preferences regarding resuscitation, ventilation, feeding tubes, and other life-prolonging interventions. Emotional and Mental Wellbeing: Guidelines for emotional support, including therapy, support groups, and the presence of loved ones during your final days. Spiritual/Religious Needs: If applicable, your directive will include any spiritual or religious rituals, prayers, or customs you'd like honored. Funeral or Memorial Plans: Space for any specific instructions on how you'd like your life to be celebrated after passing, including desired funeral rites or memorial services. Two Rounds of Revisions: We understand that your thoughts and wishes may evolve over time. Cancellation Policy
One Month of Follow-Up Support: In addition to the initial week of follow-up communication included in the Basic Package, this premium package offers one month of follow-up support via phone or email. This gives you ample time to reflect, ask any questions, and make any adjustments to your directive, ensuring it continues to meet your evolving needs. Personalized Care Coordination: We will work with your healthcare providers, family members, or any other professionals you wish to involve, ensuring that everyone understands and is aligned with your advanced care directives. We can help facilitate conversations with your loved ones, helping them to be part of this important process. Legacy Planning Session (Optional Add-On): For those who wish to further clarify their legacy, we offer an optional legacy planning session. This can include guidance on writing personal letters, creating memory books, or any other means of passing on your values and wishes to your family. End-of-Life Resource Toolkit: You will receive a toolkit with resources designed to help you navigate the end-of-life process.
